It is extremely important that you let us know if you have changed your name, home address, phone number or email address. This ensures that we can contact you when necessary and that any correspondence is sent to the correct person/address.
You can inform us of change of details through the following methods:
- In person - Please let a member of our Reception Team know if any of your personal details have changed. You can do this by approaching our reception desk.
- By post - If you would like to post your new details to us, please send them to the practice address which can be found HERE. Please remember to include your full name, date of birth, NHS number (if you have it available), your address and contact number.
